We are a small multidisciplinary team serving adults residing within the Borough of Harrow. The service operates seven days a week between the hours of 9am and 5pm (subject to review) and is supported with an out of hours on-call facility between 5am and 9pm.
The team focuses on users with severe and persistent mental disorder who have a background of multiple hospital admissions and have difficulty accessing mainstream mental health services.
All contacts take place in users' own homes, public places or other mutually agreed venues.
Referrals are normally received from care co-ordinators from one or more of the three community mental health teams:
