Our diabetes services manage the care of patients with complex or difficult to control diabetes.
Services aim to support people with diabetes to self-manage their condition. They work with general practitioners (GPs) and practice nurses to identify people needing additional support, and provide services in home, community and hospital settings.
Diabetes specialist nurses also support carers and those involved in the care of patients with diabetes, for example, staff in nursing or residential care homes.
