CNWL services are part of the launch of a new integrated triage system linking eight urgent community service response teams covering Hounslow, Hillingdon, Brent, Harrow, Ealing, Kensington and Chelsea, Westminster, Hammersmith & Fulham.

CNWL clinicians will be part of a rota managing referrals from 111 / 999 and the London Ambulance Service to streamline the system of redirecting appropriate patients to teams working hard to prevent avoidable hospital admissions, and align practices so that patients get the same high quality experience of care wherever they live.

All of the North West London teams are supporting this new Single Point of Access which is hosted by West London NHS Trust.

As we work increasingly across place with other providers, this is a great opportunity to build on those relationships for the benefit of the people in our local communities.

Thanks also to David Tudor, the Business Manager for CIS, who worked hard with partners to support the launch.
