Brent Assertive Outreach Team supports service users who experience symptoms of psychosis and who find it difficult to engage with services.

This may be because they are homeless or street homeless, have drug and alcohol difficulties, have been involved with the criminal justice system or have complex difficulties.

Service users referred will usually have had a history of admissions to hospital, possibly detained under the Mental Health Act and often encountered problems with medication treatments.

Eligibility criteria (who is the service for?)

The team provides support for people aged over 18 who have a primary diagnosis of severe and persistent mental disorder, associated with a high level of disability, multiple complex needs and who have difficulty maintaining lasting and consenting contact with services.

Service users will already have an allocated care co-ordinator directly linked to either a community recovery team or an early intervention service.

How can someone be referred?

Referrals to Brent Community Outreach Team are made by professionals/clinicians who have had current or previous engagement with the service user. The service is unable to accept self-referrals or provide a walk-in service.

The team also accepts directly referrals by an out-of-area community outreach team if the service user has recently moved to the area. 

Can I use NHS e-Referral (formerly Choose and Book) to arrange my first outpatient appointment?

The team is currently unable to accept referrals via NHS e-Referral.

Service times

The service is available from 09:00-17:00, Monday to Friday.
