Eligibility criteria (who is the service for?)

The service is for adults over the age of 18 who are registered with a Camden general practitioner (GP) who have a diagnosis of type 2 diabetes and one or more of the following:

  • Require injectable therapies
  • Insulin-treated type 2 diabetes patients with poor control
  • Problematic hypoglycaemia
  • Type 2 group education on new diagnosis and ongoing (DESMOND)
  • Step-down care from hospital (renal/diabetes)

How can someone be referred?

The service accepts referrals from GPs and other healthcare professionals. Referrals should be made to the Camden Integrated Care Service via the Camden Clinical Assessment Service: email cics.referrals@nhs.net or telephone 020 3317 3576.

The service also accepts self-referrals from patients and their carers for type 2 diabetes group education. To make a referral, telephone 020 3317 2438.

Service times

The service is available from 09:00-17:00, Monday to Friday (excluding bank holidays), by appointment only.

Services are delivered at St Pancras Hospital (diabetes hub), and a number of health centres across the London Borough of Camden. Services can also be provided at home if required.
