Care Connection Teams (CCTs) look after patients aged 65 years and over, who have been identified by the GPs as needing case management because of their health and social care needs. Where appropriate, a care plan will be discussed and agreed with the patient on how best to manage their needs. There are a total of 15 CCTs across Hillingdon.

CCTs work closely with the following partner organisations:

  • Central and Northwest London Foundation Trust
  • The Hillingdon Hospitals
  • H4ALL
  • Hillingdon GP Confederation
  • London Borough Of Hillingdon
  • Other local and surrounding hospitals.

About the team

The CCT team includes:

  • GP
  • Guided Care Matron
  • Care Coordinator.

The CCT will also work closely with other partners such as care of the elderly consultants, rapid response, district nurses, social services and H4All wellbeing officers. 

CCTs will focus on:

  • Reducing hospital admissions and A & E attendance
  • Reducing hospital visits and outpatient follow ups
  • Improved use of services for individual patient needs
  • Working together with local partners including:
    • Hospital
    • CNWL (district, community nursing and rapid response)
    • GPs
    • Third Sector
    • Social Services
    • Patients
    • Carers
  • Improving patient quality of life
  • Supporting patients to manage their own care
  • Caring for people in their home for longer.

Eligibility criteria (who is the service for?)

The service is for patients aged 65 years and over, including those in their normal place of residence, such as care and nursing homes, and supported living.

How can someone be referred?

The patient must be referred by a healthcare professional, who will state the specific needs of the patient that the CCT needs to be aware of. Patients must be registered with a Hillingdon GP to access the service.

To provide holistic care, it is important for the CCT to be able to share clinical information with those involved in the patients’ care and it would be helpful to have consent from the patient to do this. As the CCT relies on data sharing, not doing so will limit the care the CCT is able to provide to the patient.

Can I use NHS E-Referral (formerly Choose and Book) to arrange my first outpatient appointment?

We accept electronic referrals through the generic email addresses for each CCT. These details will readily be available to all our partners.

Service times

Services are provided between 9am – 5pm, Monday to Friday and deliver a coordinated approach to patient care.

Please note that the CCT is not an emergency service, for general queries, please contact your GP as usual, and for out of hours please contact 111. For more urgent/ life threatening emergencies, please contact the emergency services.
