This service is provided for local Harrow residents who are suffering with mental health difficulties.
Personal Budgets are provided to individuals to meet the cost of their social care packages. We assess a person’s eligibility for a Personal Budget under the Care Act 2014. Following a Care Act 2014 assessment, we collaboratively work on a support plan with the service user. The support plan is presented to the Resources Allocation Panel for funding. This is also subject to a financial assessment by the Local Authority to determine the service users contribution (if applicable). The Personal Budget and support plan is reviewed annually.
Eligibility criteria (who is the service for?):
Harrow residents aged 18 to 64 who require a personal budget to meet their social needs and are not on the Care Programme Approach (CPA) or do not have a care coordinator.
A person under CPA can be assessed for a Personal Budget by their care coordinator
How can someone be referred?
- Self-referral via GP or Single Point of Access (SPA)
- Their psychiatrist or health professional
