Hillingdon Community Heart Failure Nursing Service provides specialist nursing advice and management of adult patients with a diagnosis of heart failure.

Eligibility criteria (who is the service for?)

The service is for patients diagnosed with heart failure who are under the care of their general practitioner (GP). They must also:

  • Have a diagnosis of left ventricular systolic dysfunction (LVSD) with objective evidence such as echocardiography, and
  • Be willing to accept the added support of the service

Discharged patients can also refer back into the service.

How can someone be referred?

The service accepts referrals from a variety of primary care settings including GPs, district nurses, community matrons, and secondary care, cardiologists, care of the elderly, post-discharge or from obstructive pulmonary disease (OPD) clinics.

Referrals are initially discussed with the Heart Failure Nurses to ensure appropriateness and then information is faxed to Oak Farm Clinic on 01895 484811.To discuss a referral with the Heart Failure Nurses, telephone 01895 484810 .

Can I use NHS e-Referral (formerly Choose and Book) to arrange my first outpatient appointment?

NHS e-Referral is not applicable to this service.

Service times

The service is available from 8.30am to 4.30pm, Monday to Friday.

It is  provided at a number of clinics in the London Borough of Hillingdon.

The service makes domiciliary visits by prior arrangement at the patient's home address.
