Milton Keynes Community Occupational Therapy Service supports adults with physical disabilities in Milton Keynes to live as independently as possible.

The team comprises occupational therapists, occupational therapy assistants and administration support. Technicians are available to the service to provide minor adaptations.

The occupational therapist will make a full assessment of the individual’s needs. Depending on the outcome of the assessment, they may provide advice, equipment or make recommendations for changes to the individual’s living environment or recommend re-housing.

The Community Occupational Therapy Team works in a wide range of community settings including individuals’ homes, nursing and residential homes. The team may visit individuals and their carers at home to review their progress.

In addition, we provide occupational therapy services in hospital settings, including inpatient services at Bletchley Community Hospital, day services at Milton Keynes Rehabilitation Unit and acute services at Milton Keynes General Hospital.

Eligibility criteria (who is the service for?)

The service is offered to individuals who are aged 18 and over, live in the Milton Keynes Unitary Authority area and have any of a wide range of long-term physical disabilities. Some service users may also have underlying mental health issues.

Individuals are assessed within their home and our service will be offered to those who have a critical or substantial need for help.

How can someone be referred?

The service accepts referrals from anyone, including health care professionals, patients, families and their carers. To make a referral, telephone the Adult Social Care Access Team on 01908 253772.

Please note, we cannot take referrals on the office number.

Can I use NHS e-Referral (formerly Choose and Book) to arrange my first outpatient appointment?

NHS e-Referral is not applicable to this service.

The service is available from 09:00-17:00, Monday to Friday (excluding bank holidays).
