Milton Keynes Assertive Outreach Team works with service users who are difficult to engage and need help with activities of daily living or social inclusion. This may be because they are not taking their prescribed medication or may need motivation to attend activities.
The team provides support for people aged over 18 who have a primary diagnosis of severe and persistent mental disorder associated with a high level of disability, multiple complex needs and who have difficulty maintaining lasting and consenting contact with services.
Service users will already have an allocated care co-ordinator directly linked to either a Community Recovery Team or an Early Intervention Service.
Referrals to the Milton Keynes Assertive Outreach Team are made by professionals/clinicians who have had current or previous engagement with the service user. The service is unable to accept self-referrals or provide a walk-in service.
Referrals are also accepted directly to the team by an out-of-area community outreach team if the service user has recently moved to the area.

The service is available 9am to 5pm, Monday to Friday and at weekends and bank holidays from 10am to 6pm.
