Camden Neurological and Stroke Service provides specialist neurological rehabilitation to people living in Camden. The team sees patients with any neurological diagnosis including, MS, Huntington's, brain injury and stroke and works with patients in a range of community settings including their own homes, gyms, hospitals or clinics. The team works closely with social services and the voluntary sector to make sure that patients' long-term needs are met.

The team consists of physiotherapists, occupational therapists, neuropsychologists, speech and language therapists and rehabilitation assistants. Patients can be referred to one or more disciplines within the team.

Comprehensive rehabilitation programmes are offered for people with neurological conditions. Programmes last for up to eight weeks. The team supports people with physical, cognitive and emotional difficulties as a consequence of their condition. The aim of the rehabilitation programme is to optimise people’s independence and function.

The navigation service supports patients with complex neurological diagnoses and helps facilitate transitions between hospital, rehabilitation and home. The team works closely with patients and their families, and with health and social care professionals across the entire patient pathway to identify suitable options for local and specialist rehabilitation.

Neuroactive is a short-term exercise programme for all adults with a long-term physical or cognitive difficulty with a neurological diagnosis, who wish to transition to a community gym, or independently maintain their current physical condition.

Patients will need to arrange transport to the Peckwater Centre for assessment and therapy sessions. Medical consent to exercise is required from a patient’s GPs.

The service facilitates early discharge from inpatient stroke services and offers the same level of care in a community setting that a patient would receive in an inpatient rehabilitation unit.

We work with Carelink to offer enabling care packages when required and the service is offered for up to six weeks. A care package could include providing a carer to support daily living activities such as washing and dressing or cooking.

To be eligible for these services, you need to be living in Camden, aged over 18, registered with a GP and have a neurological diagnosis.

  • Patients who only need equipment or carers to assist them with daily life should telephone Camden Social Services on 020 7974 4000
  • Patients requiring outpatient musculoskeletal physiotherapy should telephone Connect on 020 7871 0545
  • Patients needing specialist voice therapy should be referred to the Voice Clinic at the Royal National Throat, Nose and Ear Hospital. Please telephone 020 3456 5361.

How can someone be referred?

Please provide as much detail as possible, as any missing information may delay the referral being processed. The risk assessment section is particularly important as it enables staff to work safely (visits will be conducted alone unless indicated otherwise). Healthcare professionals should attach any relevant discharge or outpatient reports to the referral form. Referrals for Stroke Early Supported Discharge:

Eligibility criteria

This service is for Camden residents aged over 18 and registered with a Camden GP. Individuals must be referred from an inpatient setting, have a new diagnosis of stroke and be able to transfer with assistance of one. The service is appropriate for patients who would benefit from intensive therapy within a community setting.

All patients must be be medically fit for discharge, and have clear rehabilitation goals, patients can can be referred to one or more disciplines within the team.

How can someone be referred?

The service accepts telephone referrals from any health professional in a primary care setting, such as hospital therapists, nurses, or consultants.
