Our children's community nursing team care for children and young people who:
- require nursing support
- are between the ages of 0 and 19.
We are a team of experienced children's nurses who work closely with other healthcare professionals to provide high quality care for your child.
Children and young people who access this service must be registered with a Harrow GP and/or should be a Harrow resident.
We care for children who need nursing support for a range of conditions, including:
- respiratory conditions, including oxygen dependency and tracheostomy care
- feeding difficulties, including the management and teaching of nasogastric and gastrostomy tube feeding
- skin care needs, such as support with dressings, eczema and burns
- chronic conditions requiring long term support, such as diabetes.
We also provide support for children who need:
- intravenous therapy or help with central lines
- palliative care.
The children’s community nursing team are registered children’s nurses who aim to provide a co-ordinated holistic approach that promotes safe practice, flexibility and the delivery of quality care to the child or young person. They work in partnership with children and young people, their families and other professionals in a range of community settings including home, school, nursery, playgroup and respite care facilities. This includes working in collaboration with the school nursing team to support the delivery of care, management of children’s nursing needs and provision of education and training within pre-school and schools settings.
Patients can access this service following a referral from a health practitioner. Following the initial referral, a named registered nurse will co-ordinate and oversee the nursing package of care based on the child or young person's needs. This will include working in partnership with families, individuals and carers to identify and meet the needs of the each group.
We accept referrals for children who:
- live in Harrow
- have a Harrow GP.
There must be an agreement that it is practical and safe to carry out the required nursing care at your child's home, school or nursery. This agreement must be made between:
- the referrer
- the family
- a GP or paediatrician
- the children's community nursing team.
