Community Living Well is a primary care mental health service for people aged 16 and over who need support with their mental health and wellbeing. It’s for people registered with a GP in the Royal Borough of Kensington and Chelsea, or the Queen’s Park and Paddington areas of Westminster.

Community Living Well offers a different kind of mental health support. It makes it easy for people, or those who care for them, to access a wide range of clinical and wellbeing services. The services on offer include talking therapies, peer support group, help with employment and support with debt, housing and benefits issues.

Community Living Well’s Self-Care programme helps people to take more control of their mental health and physical wellbeing. It helps to build social contact with access to community, leisure, education, arts and volunteering opportunities. 

The services available include: 

  • The Clement James Centre Wellbeing Programme, offering an employment, advice and wellbeing service, which includes a range of activities and support. A chance to build confidence, relax and connect with others.
  • Emotional and practical help in other languages, including Arabic, Farsi, French, Somali and Spanish, run by the BME Health Forum. 
  • Massage and reflexology to help relax and unwind, provided in partnership with Community Massage London and SMART. They are currently offering online sessions in guided self-massage and relaxation. 
  • My Recipe, My Story, an activity group for women of diverse backgrounds, helping to connect with others in the community. 
  • Volunteering on Prescription which provides opportunities to help meet people, share skills and improve wellbeing. 
  • Well Read play-reading group; a chance to meet new people and read different stories in a friendly and supportive environment.

How to refer

Community Living Well is available to anyone aged 16 and over and who is registered with a GP in Kensington and Chelsea, Queen's Park or Paddington.

GPs can refer patients via SystmOne, or patients can refer themselves by completing this online form, or by calling on 020 3317 4200.
