Hillingdon has been confirmed as one of 43 sites across the country to benefit from the government’s new neighbourhood health plans.
The borough will be among the first neighbourhood health services in the country. This national recognition supports existing local plans to roll out three primary care hubs in the South East (Hayes and Harlington), South West (Uxbridge and West Drayton), and the North (Ruislip, Northwood and Harefield).
They are designed to bring more services closer to home for our residents and will bring GP, community health services, mental health, voluntary sector, some outpatient services, diagnostics and adult social care together. This will improve access to same day primary and community care, provide proactive multi-disciplinary care for people with complex needs and help people stay well for longer by tackling health inequalities and focusing on prevention, early intervention, and wider determinants.
The hubs will give people faster access to the right care, reduce the need for hospital visits, and ensure support is available where and when our community needs it most.
Neighbourhood health services will benefit patients by providing end-to-end care and tailored support, looking beyond the condition at wider causes of health issues, helping to avoid unnecessary trips to hospital, prevent complications. Aligned with the three key shifts of the NHS 10-year plan—boosting out-of-hospital care, reinforcing personalised care, and reducing health inequalities—this transformation would reposition primary care as the cornerstone of a more integrated, preventative, and community-focused system.
CNWL is designated as the integrator organisation for Hillingdon. It provides the governance infrastructure, system enablers, and operational support that make the 'Place Operating Model' work for all partners.

I was lucky to visit The Pembroke Centre in Ruislip Manor — the first site for our new community hubs in Hillingdon. It was fantastic to see first-hand how they are pioneering this integrated approach to healthcare – bringing together health professionals from across the health service, providing more services for patients in one place, including both physical and mental health.
Providing more services in our community will not only help relieve pressure on our hospital, but also mean every resident receives the high-quality care they deserve in a more convenient way. I look forward to working closely with the local GP Confederation, Health Trusts and residents to make this first site a success and ensure we also deliver a much-needed neighbourhood health hub in Uxbridge and the South of the Borough too.”

By strengthening neighbourhood hubs, services can be delivered closer to where people live, making care more responsive to local needs and reducing reliance on hospital-based interventions. The programme builds on examples of local best practice highlighted in the 10 Year Plan, where some patients are already benefitting from a joined-up neighbourhood approach. This scheme will bring the best of the NHS to the rest of the NHS.
