The aim of the programme is to develop a new population-based model for mental health care in the community. The new model of care will redesign and reorganise the existing borough based community resource to form new Community Mental Health Hubs, each of which is aligned to the local Primary Care Networks. Over the past two and a half years, CNWL has worked on improving community mental health services for our patients and carers. Boroughs worked with service users and carers to design new hub models based on national guidelines from NHS England and NHS Improvement in the new Community Mental Health Framework. CNWL is an ‘Early Implementer’ of this new hub model.

The new model will involve replacing the old ‘Care Programme Approach’ (CPA) framework, whilst retaining good care coordination and high-quality care planning. You can read the Trust’s full position statement on the move away from CPA here.

What will the new model of care mean for service users and carers?

  • Better outcomes with more focus on treatment than on assessment  
  • Easier to access, better connected care with the service user at the centre
  • Faster access to mental health treatment when people need it to prevent relapse
  • Service designed based on what the local community needs, so care is more personalised
  • More consistently high quality of care across the community  
  • Care is recovery focussed, helping service users stay well in their community
  • Service users will create a care plan with their clinician reflecting their needs and goals.
