Referrals to the service are assessed against our referral criteria (please see drop down).

If we think we can help you, we’ll send you, and the professional who referred you, a letter of acceptance.

If we don’t think we can support your needs, we’ll send you, and the professional who referred you, a letter explaining why, and may suggest an alternative service that we think may help.

Alternatively, you may find a dental practice near you at find a dentist.

Submitting a referral form

Dental Professionals must use the online Rego Vantage system.

Other healthcare professionals please use this referral form.

How to self-refer or for others

Individuals who want to refer themselves or another person should complete a referral form.

Our service are generally only available to individuals who are referred to us by a medical or dental practitioner or other health/social care worker and individuals who fall into one or more of the following categories:

  • Learning disabilities
  • Severe mental health problems
  • Severe behavioural management problems (may include young children  requiring general anaesthesia)
  • Complex medical problems
  • Very severe anxiety and dental phobias * only for people living in Buckinghamshire – this is not available in Milton Keynes
  • Physical disabilities who either may need domiciliary care or due to their disability are unable to access a local NHS dentist

And as a result of one or more of the above, this impacts their ability to receive dental care from a General Dental Practitioner.

If an individual does not fall into any of these categories, we regret they will not be eligible to be seen by our service.
