The Telehealth Service offers a specialised nursing service for patients with long-term conditions.

Telehealth is a user friendly piece of equipment that can improve the quality of life of patients living with a long-term health condition.

Telehealth involves a small electronic unit, roughly the size of a telephone, being installed in your home. Devices are then used to take readings such as blood pressure, oxygen levels, weight, pulse and temperature, depending on your condition.

It does this by monitoring your health on a daily basis for a period up to 3 months (at the clinician’s discretion) and identifying any deterioration before it becomes critical. This can help reduce the need for you to be admitted to hospital.

Telehealth can make a real difference to your life - not only to help you maintain your independence - but also for your families and loved ones.

Many patients already using telehealth report an improvement in their overall confidence and added peace of mind that their condition is being monitored.

For people over the age of 18 who have been diagnosed as having a long-term condition of COPD (Chronic Obstructive Pulmonary Disease), heart failure or diabetes and meet one or more of the following criteria:

  • Newly diagnosed with any of the above.
  • Have had 2 or more admissions to hospital in the last 3 months for the same long-term medical condition
  • Have the ability to use equipment independently or has support from carers, family or friends
  • Capacity to self-manage condition following the education programme
  • Patients also have to be registered with a Milton Keynes GP.

People can be referred through many sources such as self-referral, through your GP, practice nurse, specialist nurse, hospital clinician, hospital, carer, or district nurse. If you wish to discuss the referral please call the above number.

Services are available from 9am to 5pm, seven days a week, 365 days a year. The Telehealth triage team will review readings seven days a week between the hours of 9am to 1pm
