We are pleased to announce CNWL and North Central London Clinical Commissioning Group (CCG) have launched a remote monitoring pilot with my mhealth for patients with the long-term condition diabetes.

The introduction of the myDiabetes app will allow patients to help manage their long-term condition with an accredited education platform, as well as enabling the remote monitoring of key indicators by clinicians. Clinicians will utilise the service to remotely support patients within their place of residence, away from a clinical setting. This is particularly useful throughout the COVID pandemic and helps patients at their most vulnerable, at a time where they may not feel comfortable visiting their GP practices, hospitals or are actively encouraged not to attend.

It also supports delivery of one outcome within the NHS Long-Term Plan, launched in January 2019, where healthcare services are asked to support patients to self-manage their long-term conditions through education, training and provision of resources and support.

The introduction of the app and remote monitoring of patients with diabetes is expected to bring about significant benefits in the streamlining of high-quality care within the community and help to:

  • build on the digital competency and capability achieved during the pandemic
  • accelerate the scale of those digital innovations that enable a remote care service
  • help save staff time
  • reduce the need for non-elective admissions
