The Hillingdon Neurology and Stroke Rehabilitation Service is provided by Community Adult Rehabilitation Services. 

The Neurology and Stroke Rehabilitation team consists of physiotherapists, occupational therapists, speech and language therapists, rehab support workers and a dedicated team administrator.

The team supports patients with advice on tailored exercise programmes, equipment and adaptations to enable them to work towards their functional needs and goals. They visit patients in a range of community settings including their place of residence, and have a day centre and outpatient clinic at Eastcote Health Centre.

Neurological rehabilitation in the community is provided by a team of Physiotherapists, Occupational Therapists and Speech and Language Therapists and Rehab Support Workers with specialised skills in assessing and treating patients with neurological conditions.

Depending on your needs, the rehabilitation programme can be delivered at your residence, day centre or in our clinic at Eastcote Health Centre. The team can provide equipment, tailored exercise programmes, adaptation and advice to support you to work towards your functional needs and goals.

Neurological conditions include stroke, ataxia, motor neurone disease, multiple sclerosis, brain injuries, Parkinson’s disease, Huntington’s disease and spinal injuries.

A stroke is a serious life-threatening medical condition that happens when the blood supply to part of the brain is cut off. Stroke is a medical emergency and urgent treatment is essential. The sooner a person receives treatment for a stroke, the less damage is likely to take place.

The Stroke Early Supported Discharge (ESD) service is a collaborative approach in stroke rehabilitation between CNWL, Hillingdon Hospital, The Stroke Association, Social Services, and H4ALL in Hillingdon to facilitate hospital discharge and immediate access to specialist rehabilitation for stroke survivors in the community.

Referrals are accepted only from hospital admissions. Following discharge from hospital and the referral being accepted by the ESD service, our team of specialist Physiotherapists, Occupational Therapists, Speech and Language Therapists, Psychologists and Rehab Support Workers, work with a Stroke Consultant and Specialist Stroke Nurse at Hillingdon Hospital to deliver a personalised rehabilitation programme for patients at their home/place of residence. We work with patients most days of the week to support them in achieving their agreed rehabilitation goals.

 

Parkinson’s disease (PD) is a progressive condition which can result in the loss of the smooth and controlled movement of your muscles and joints as well as causing problems maintaining a good posture, balance, general mobility and confidence. This can limit some activities of daily living.

The positive benefits of following a regular exercise regime include maintaining your abilities, strengthening your muscles, increasing mobility in your joints and building up general fitness and health. This will improve your capability to keep independent for as long as possible. Exercise is also good for your general wellbeing and mood. But remember your condition may alter if you are tired, if your Parkinson’s is more pronounced, or if your medication is altered.

Our team are highly skilled, experienced clinicians who are here to support you along your Parkinson’s journey.  We run the PD get up and go classes for people recently diagnosed or those who are further along their Parkinson's journey.

This service is for Hillingdon residents over the age of 18 with any neurological diagnosis . 

The service welcomes self-referrals and referrals from families, carers, general practitioners (GPs), other health and social care professionals and voluntary agencies.

The service is available from 8am to 5pm, Monday to Friday (excluding bank holidays).

The service visits people in their own home and provides outpatient neurological rehabilitation at Eastcote Health Centre for patients who are able to attend the clinic in person.
