ESRT is a multidisciplinary team of professionals who are specifically stroke trained. The team consists of a co-ordinator, nurse, psychologist, speech and language therapists, occupational therapists, physiotherapists, rehabilitation assistants and an administrator. The team accept referrals for anyone who has a new stroke diagnosis.
Rehabilitation is provided in your own home or care home setting. The team aims to support you with your rehabilitation goals, whether they are to regain or to maintain skills and independence in activities of daily living, or other activities that are meaningful to you.
We can support with:
- Adjusting to the emotional consequences of having a stroke
- Mobility difficulties
- Difficulties in day-to-day functional activities
- Communication skills
- Strategies to help with changes in cognition or memory
- Safe swallow with food and drink
- Other self-management strategies
The team will assess you once you have been referred into the service; this is usually on discharge home from hospital or after you have been seen in an outpatient clinic.
Depending on the level of need and with your consent, you may be seen for up to six weeks, though some people may only require a one-off visit.
The service is available to people over the age of 18 who have had a new stroke and are registered with a Milton Keynes general practitioner (GP)
Individuals are referred into the service by their hospital consultant or GP.

What to expect?
A member of the team will aim to contact you by telephone within two working days of receiving the referral. If you do not hear from us, please contact us on 01908 724 599.
The team will visit you at your home. The initial appointment will be to gather more information, introduce the service and identify your goals for therapy.
