Neuro Conditions Service

Working together to support people to live well with their neurological condition, to enhance quality of life and wellbeing by achieving the things that matter to them

If you need help in a crisis, please do the following:

  • Contact the CNWL service looking after you during their office hours
  • For urgent or emergency care please contact the NHS services found on this page
  • If you are not already being looked after by CNWL or elsewhere within the NHS, see your GP during normal working hours as they can then refer you to a local service

About our service

The Neurological Conditions Service (NCS) consists of the Early Stroke Rehab Team (ESRT), the Neurological Rehabilitation Service (NRS) and the Neurological Clinical Specialist Team (NCST).

The teams work in partnership with people who have an acquired or long-term neurological condition and their families and / or carers.  They carry out specialist, person centred assessments and work towards meaningful goals, to facilitate independence and promote safety and function.  They provide specialist advice, support and education to enhance quality of life and empower people to self-manage their condition.

The teams work alongside their colleagues in health and social care and voluntary services to make sure that care is maintained in the community setting.  Their support includes preventing and supporting discharge to and from secondary and tertiary care settings.
