Milton Keynes Neurological Rehabilitation Service is a specialist community-based team who provide assessment, advice, support, management and/or rehabilitation to adults with acquired or long-term neurological conditions aged 18 years or over, with a Milton Keynes GP.
The service is provided by a specialist multidisciplinary team of NHS professionals including physiotherapists, occupational therapists, speech and language therapists, psychologists and rehabilitation assistants.
We work in partnership with people with neurological conditions, their families and / or carers. The team provide time-limited input, supporting people to work towards meaningful goals, facilitating independence, promoting safety and function, enhancing quality of life and empowering them to self-manage their neurological condition.
Depending on the type of therapy intervention and the agreed goals, the team will offer appointments either at home, in the community or in the outpatient unit. Virtual consultations by phone or video can be offered where this is appropriate. We work in collaboration with other services such as other health-care teams, leisure/sports facilities, social services and the voluntary sector to provide person-centred care.
The service is for anyone aged 18 years or over who is registered with a Milton Keynes general practitioner (GP) with an acquired or long-term neurological condition.

What to expect?
When we receive a referral, the team will check whether our service is the appropriate one for you. This might involve contacting you or the referrer for more information. You will be sent a letter acknowledging the referral and providing our contact information. There is a waiting time for appointments.
When a space becomes available, we will contact you by letter or telephone to offer you an initial appointment. An initial assessment appointment usually lasts up to 90 minutes. You will meet with one or two members of the team, this may include a student. This initial appointment could be on the telephone, in person at our site in Bletchley, at your home or another agreed site.
At the first meeting you will have time to explain your challenges relating to your neurological diagnosis, ask questions and talk about your goals and hopes. The therapist will talk with you to understand more about what is difficult, what is going well and what is important to you.
At the end of your initial assessment, we will agree a plan of what to do next. Sometimes people just attend for an assessment and no further therapies are planned. Sometimes assessment occurs over a few meetings. We always aim to explain what is happening and agree the goals and plan with you.
