This is the full statement we made to the programme following the death of Mr Tariq Dalton  in 2018.

Statement to File on Four

CNWL deeply regrets the death of Mr Dalton in 2018 and the distress caused to his family as heard on your programme tonight.

Inquiries into Mr Dalton’s death have been thorough; with a police investigation, an Inquest, a Prison Ombudsman Report and our own investigation.

We accepted the Ombudsman’s recommendations and system changes have been made which have improved healthcare; actions taken on the recommendations are published by the Ombudsman. These include improved communication between health professionals in physical and mental healthcare teams on complex patients, and  use of electronic systems which issue clinical alerts and reminders to prevent prescribing errors.

On PPI we can only repeat what the Ombudsman said (at para 86):

We agree and are saddened to say so.

Change has occurred from the very sad circumstances of Mr Dalton’s death. And we repeat our regret to Mr Dalton’s sister.
