A video and article aimed at helping people understand more about Advance Care Planning (ACP) has been published.

The project was a collaboration between CNWL, the Open university, Milton Keynes University Hospital, and the Willen Hospice; its aim to provide information in relation to Advance Care Planning, what it means, why it is important and where to get further information.

ACP is the process of thinking about, documenting and sharing one’s wishes, beliefs, values and preferences for one’s future care, often captured in a document called an advance care plan.

The plan and the conversations around it can be used to help align and coordinate care around these preferences and means that the care people receive towards the end of their life is more likely to be the kind of care they want.

CNWL’s Neurological Conditions Clinical Specialist in Milton Keynes, Anna Kent, was heavily involved in getting the project off the ground and in helping find people willing to be filmed.

Anna says: “This was a passion project for me because the ACP means so much to me and the people I work with.

“It was a privilege to have been part of this exciting collaborative project, which provides a medium to highlight the importance of advance care planning.”
