On 9 July, pharmacy colleagues from Central and North West London NHS Foundation Trust (CNWL) joined the London Region Discharge Medicines Service (DMS) Improvement Workshop.

Meenal Patel and Jackie Box, CNWL pharmacy leads gave a presentation about our quality improvement work, sharing how we’ve embedded DMS across the Trust. They spoke about what’s worked, what we've learned, and the difference it’s making for patients.

The workshop brought together NHS Trusts, integrated care boards, general practice and community pharmacy leaders. Together they explored how better coordination can make care safer.

Thanks to the commitment of CNWL’s pharmacy teams, our patients are experiencing smoother and safer transitions from hospital to home.
