The Home First team – Transforming Discharge Destinations for Patients with Cognitive Impairment Following a Period of Intermediate Care team has been recognised for their exceptional work, receiving a Highly Commended award at the prestigious HSJ Patient Safety Awards last night.

The team was acknowledged in the Quality Improvement Initiative of the Year category for their innovative approach to improving discharge outcomes for patients with cognitive impairment after intermediate care. Their work has focused on creating safer, more person-centred discharge pathways, ensuring patients receive the right support at the right time.
